Submit all the required documents to the Student Affairs & Services – Scholarship Unit:

Certificate of Residency (Dagupan City): 1 original + 1 photocopy

Student Load / Proof of Enrollment: 1 original + 1 photocopy

Certificate of DSWD (if listahanan or 4Ps member)

Deadline of submission: October 31, 2025

Where to submit: Scholarship Unit, Window 2

Note: Please make sure your enrollment load is readable and certificate of residency must be sealed and must be duly-signed by the brgy. captain. Late or incomplete submissions may delay or disqualify your application.

Submission of these documents does not automatically guarantee approval — Final evaluation, validation, and slot availability will still be considered by UniFAST / CHED.
