Your subsidy for the Academic Year (A.Y.) 2024-2025, 1st and 2nd semester, has already been TRANSFERRED to the accounts of Lyceum-Northwestern University (L-NU).

Students whose names appear in the attached schedule are REQUIRED TO SIGN the “received payments” documents so the subsidy can be credited directly to your tuition fee payables.

Required Documents

Please BRING the following requirements:

Four (4) Photocopies of the A.Y. 2024-2025 School ID on SHORT BOND PAPER (8.5 x 11 in) ONLY with 3 Specimen signatures in BLUE INK.

Bring your own BLUE BALLPEN.

For Representatives (Immediate Family Members ONLY):

The representative must bring the following:

a. Notarized Authorization Letter-one (1) original and three (3) photocopies.

b. Four (4) Photocopies of the A.Y. 2024-2025 School ID of the student on SHORT BOND PAPER (8.5 x 11 in) ONLY with 3 Specimen signature in BLUE INK.

c. Four (4) Photocopies of any valid ID of the representative with 3 specimen signatures in BLUE INK ONLY.

d. Four (4) photocopies of the Birth Certificates of both the student and representative.
