Please BRING the following requirements:

โœ…Four (4) Photocopies of the A.Y. 2024-2025 School ID on SHORT BOND PAPER (8.5 x 11 in) ONLY with 3 Specimen signatures in BLUE INK.
โœ…Bring your own BLUE BALLPEN.
โœ…For Representatives (Immediate Family Members ONLY):

The representative must bring the following:
a. Notarized Authorization Letter-one (1) original and three (3) photocopies.
b. Four (4) Photocopies of the A.Y. 2024-2025 School ID of the student on SHORT BOND PAPER (8.5 x 11 in) ONLY with 3 Specimen signature in BLUE INK.
c. Four (4) Photocopies of any valid ID of the representative with 3 specimen signatures in BLUE INK ONLY.
d. Four (4) photocopies of the Birth Certificates of both the student and representative.
